Clinical Considerations in Prehospital Mechanical Ventilation for Trauma Patients.

Managing trauma patients in the prehospital setting requires rapid, evidence-based decision-making, particularly when mechanical ventilation is needed. Trauma-related respiratory compromise can result from airway obstruction, pulmonary injury, or neurological impairment affecting the respiratory drive. Mechanical ventilation plays a key role in stabilizing these patients by maintaining oxygenation, supporting ventilation, and ensuring hemodynamic stability. However, its application must be carefully adapted to the patient’s clinical condition, anticipated transport duration, and overall injury severity.
A lung-protective ventilation strategy is essential, incorporating appropriate tidal volumes, maintaining adequate oxygenation, and minimizing the risk of ventilator-induced lung injury. Patients in hemorrhagic shock, metabolic acidosis, or thoracic trauma require individualized ventilation adjustments to prevent further physiological deterioration. Special considerations are necessary for patients with rib fractures and pulmonary contusions, as improper ventilation settings can exacerbate respiratory distress. In those at risk for pneumothorax, close monitoring is essential, as positive pressure ventilation may worsen the patient’s condition and necessitate immediate intervention.
Prehospital mechanical ventilation requires continuous reassessment, with adjustments made based on real-time clinical response and physiological monitoring. New advances in transport ventilator technology continue to refine ventilation strategies, improving both safety and patient outcomes.
